ESPECIAL VMA 1991: A bunda de Prince, REM e a estreia de Mariah Carey.

Chegamos em 1991. O Video Musi Awards era apresentado pela quarta e última vez por Arsenio Hall, mais uma vez no Anfiteatro Universal, em Los Angeles.

Neste ano, a MTV efetuou algumas mudanças na premiação. Foi acrescentado a categoria "Best Long Form Vídeo" para premiar álbuns visuais;entretanto, nunca mais foi entregue até ser anunciado sua volta em 2016. O prêmio por "Melhor Vídeo Moderno", por exemplo, foi renomeado para "Melhor Vídeo Alternativo". 

Durante a premiação, a MTV homenageou Michael Jackson dando o nome do astro pop à uma categoria. O Video Vanguard Awards passava a se chamar Michael Jackson Video Vanguard Award, devido a revolução que o cantor causou na indústria musical e na cultura popular com seus videoclipes. Graças a Michael, artistas eram influenciados e descobriam que era possível dar vidas a histórias em vídeos curtos.

R.E.M levou o maior número de prêmios. A banda recebeu dez indicações pelo clipe de "Losing My Religion" e terminou a noite levando seis astronautas, incluindo "Vídeo do Ano".

A polêmica da noite ficou por conta de Prince. Vestindo um macacão, que deixava toda sua bunda à mostra, o cantor e o dançarinos simularam uma orgia durante a performance de "Gett Off". Além de Prince, Mariah Carey e Paula Abdul também se apresentaram.
